Due to a misarranged wiring harness the stop lamp circuit and shift solenoid circuit may have their couplers interchanged when connected to the stop lamp switch. This may result in the stop lamp circuit receiving a lower than specified voltage and the shift solenoid circuit receiving a higher than specified voltage. This higher voltage fed through the shift solenoid circuit contacts within the stop lamp switch may cause the switch casing to melt, rendering the stop lamp inoperable.

During the initial charging of the CNG fuel tank, by lowering the temperature of the gas, cracks developed in the soluble alloy of the safety valve. Subsequent charging of the fuel tank in the field resulted in the cracks becoming penetrating cracks. If a vehicle is used under such conditions it is possible that gas can leak from the safety valve.

In the worst case scenario, due to the high pressure in the system and the specific engine installation, oil may be sprayed onto hot surfaces potentially resulting in smoke formation or ignition of oil, possibly leading to engine bay fire. As the oil leakage is substantial a sudden loss of steering assistance may occur.
